TO: Member Participants, Ironworkers Local No. 55
Ironworkers Local No. 55 and
Contributing Employers have established benefits supplementing
Ironworkers’ health coverage under the Union Construction Workers
(UCW) Health Plan. This
booklet describes these supplemental benefits, and is part of your UCW
Health Plan and
Summary Plan Description.
Two supplemental benefits are available:
1. Personal Account Plan (PAP)
Contributions received from employers for PAP benefits will be deposited
directly to the active
member’s PAP Account. The Account may be used to maintain your
health coverage when you
receive a notice that your eligibility is being terminated because contributions
on your behalf are
not enough to maintain your eligibility. It may also be used to obtain
reimbursement for health
benefits that are not paid by any health insurance program covering you,
your spouse, your
dependent, and which are allowable under the Internal Revenue Code. Spouses
and
dependents of deceased employees may continue to be reimbursed for medical
care expenses
up to an amount equal to the unused reimbursement amount remaining at
the time of death.
The claim form, together with an EOB and/or detailed bill, must be submitted
to receive a
reimbursement.
2. Pooled Eligibility Assistance Plan (EAP)
The EAP Plan provides assistance to certain Ironworkers in making their
self-payments to
maintain eligibility under the UCW Health Plan:
- Certain Ironworker Retirees who meet an income test provided for
in the Plan,
- Certain Non-Retired, Disabled
Ironworkers who are under Workers’ Compensation
Payment Programs, and
- Certain Active Ironworkers who may be required to make a self-payment
to maintain coverage in the UCW Health Plan because a delinquent
employer has not submitted required contributions to the UCW Health
Plan.
Except for Retiree assistance, Eligibility Assistance shall be limited
to 36 months.
Please refer to your UCW Health Plan and Summary Plan Description for
other terms which
may apply to your EAP and PAP benefits.
